Choosing an AI EMR Platform in 2026: What Actually Matters

The EMR market in 2026 looks different from the market in 2022 in one specific way. Almost every vendor now claims to be an AI-powered platform. Some of them genuinely are. Some have added a few AI features and rebranded. A few are claiming AI capabilities they don’t actually have. Telling them apart is harder than it should be.

For a practice making an EMR decision now, the AI question isn’t optional. The clinicians will expect AI-assisted documentation. The billing team will expect AI-assisted coding. The administration will expect AI-assisted operational insights. An EMR without these capabilities will feel dated within twelve months. An EMR that markets AI capabilities but doesn’t deliver them will feel worse.

So how do you actually evaluate an AI EMR Platform versus a platform that’s just calling itself one?

The first question to ask is about ambient documentation. This is the AI feature with the most operational impact in current clinical workflows. The question to ask is specific: “Show me a demo of a typical primary care visit, with audio capture, real-time draft note generation, and the clinician’s edit workflow before signing.” Then watch how it actually works. Pay attention to the latency. Pay attention to the structure of the draft note. Pay attention to how easy it is for the clinician to edit. Pay attention to whether the note reflects the patient’s actual clinical context or just the words said in the room.

The platforms that handle this well will have a confident answer and a smooth demo. The platforms that don’t will have explanations about how the feature is in beta, or coming in the next release, or requires a third-party integration.

The second question to ask is about coding assistance. After a visit is documented, the AI should propose appropriate billing codes based on the documentation, the diagnoses, the procedures, and the time spent. The clinician reviews and approves. The good platforms have this working. The marketing platforms wave hands.

The third question is about operational analytics. An AI EMR platform should be doing something with the data it captures. Surfacing care gaps. Flagging at-risk patients. Identifying revenue cycle issues. Spotting workflow bottlenecks. Ask to see the analytics that come standard. Not the custom-built dashboard the vendor put together for the demo. The standard out-of-the-box reporting and AI-generated insights that any customer gets.

The fourth question is about customisability. AI features that work perfectly for one practice’s workflow often don’t work for another’s. Pediatrics is not internal medicine. Behavioral health is not orthopedics. A walk-in clinic is not a multi-specialty group. The AI features need to be tunable to the specific practice context. Templates need to be adjustable. Order sets need to be configurable. Automation rules need to be editable by the practice, not just by the vendor.

The fifth question is about data isolation and learning. When the AI processes the practice’s data, what happens to that data? Does it train models that benefit other customers? Does it stay within the practice’s tenant? What are the contractual terms? Different vendors handle this differently and there isn’t a single right answer, but the question needs to be asked and the answer needs to be acceptable to the practice’s legal and compliance posture.

The sixth question is about pricing structure. AI features in EMRs are still settling into stable pricing. Some vendors include them at no additional cost. Some charge per provider per month. Some charge per encounter. Some charge a base fee plus usage. Read the pricing carefully. Project it out over three years assuming the practice grows. Compare apples to apples, which is harder than it sounds because the bundling varies a lot.

A few specific signals that suggest the AI capabilities are real:

The vendor can produce video of their AI features in actual use at customer sites, not just in their demo environment. Practices that are happy with the AI are usually willing to be filmed.

The vendor has published outcome data on the AI’s impact. Average documentation time saved. Coding accuracy improvements. Care gap closure rates. Specific numbers, not vague testimonials.

The vendor’s engineering team will answer technical questions about how the AI works. Where the inference runs. What model architecture is used. How updates are deployed. How edge cases are handled. Vendors that have built the AI in-house tend to answer these questions. Vendors that have wrapped someone else’s AI tend to deflect.

The clinical leadership at the vendor (chief medical officer, clinical advisory board) talks about the AI in concrete clinical terms, not in marketing language. They describe what the AI does in the exam room, how clinicians use it, what the failure modes are, what the limitations are.

A practice that goes through this kind of evaluation will end up with two or three platforms on the shortlist. From there the decision usually comes down to fit with the practice’s specific specialty, the vendor’s customer support quality, and the migration cost from the current system. The AI capability question gets answered in the evaluation. The other questions get answered in the references and the contract.

The market is going to keep evolving. In two years some of today’s category leaders will look dated. Others will have consolidated their position. The risk of picking the wrong platform is real but manageable. The bigger risk is picking a platform that doesn’t have a credible AI roadmap at all, because that’s the platform you’ll be migrating off in three years regardless. Better to make the decision deliberately now than to make it under pressure later.
